Technical Assistance:
A review of the center's safe sleep policy determined it is not consistent with safe sleep practices. The center was provided with a copy of Safe Sleep 365 flyer which states, in part, infants are to sleep apart from blankets. It is recommended that the safe sleep policy be consistent with this safe sleep practice.

Comments:
The licensing inspector initiated an unannounced inspection on 7/31/23 and concluded on 8/7/23 in response to a complaint received by the department on 7/27/23 for allegations regarding supervision and safe sleep practices. Interviews and observations were conducted relating to the allegations. The information gathered during the investigation support the allegation in part. Areas of non-compliance are identified on this Violation Notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on a review of the center's policies and interview, the center staff failed to follow the written procedures for safe sleep practice an sudden infant death syndrome.

Evidence:

1. The center's policy and procedures for safe sleep and SUIDS prevention states the following:
- Infants will always be place on their backs to sleep unless there is a signed sleep position medical waiver on file. In that case, a notice will be posted on the infant's crib.
- Infants will not be left in a rocker, high chair or similar item to sleep.
2. Staff 1 stated on 8/7/23, they did allow children to sleep in swings until recently. It would depend on what the parents wanted.
3. Staff 2 stated on 7/31/23, they are working on moving children into cribs to sleep and out of swings.
4. Staff 3 stated on 7/31/23, some parents prefer their child to sleep in swings but they are working on getting them to sleep in cribs now.

Plan of Correction: Our infant and toddler staff will complete the most recent Safe Sleep training provided through Better Kid Care on Demand. Safe Sleep Policy will be reviewed at staff training on August 12, 2003. Also, swings were removed from the classroom.
