Comments:
An unannounced renewal inspection was conducted on August 10, 2023 from approximately 11:50 am to 2:45 pm. Five staff were present supervising twenty-six children ranging in age from infants through school age. Children were observed during naptime. Six staff and five child records were reviewed. Violations are noted on the Violation Notice.

Violations:
Standard #: 8VAC20-780-40-M
Description: Repeat Violation
Based on observation, facility did not ensure there a current list of all children's allergies, sensitivities and dietary restrictions documented in each room or area where children are present. The list is to be dated.

Evidence: Allergy lists in each room were not dated. Staff stated that one child noted on the allergy list is no longer enrolled in the program.

Standard #: 8VAC20-780-210-A-4
Description: Based on review and interview, facility did not ensure when a staff member is designated as program leader that they meet the qualifications for the position.

Evidence: The record for staff #5 (start date 5/24/2023) designated the individual as "Teacher". Program Director stated that is the equivalent of program leader. The training provided in the staff record had documentation of 12 hours of training completed in 2023. Additional training included in the program leader qualifications were completed in 2021. Program Director stated she thought training from previous years could be combined with current training to meet qualifications.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
Based on observation, facility did not ensure areas and equipment of the center is maintained in a clean, safe and operable condition.

Evidence: The following was observed inside the facility:
1. In the doorway between the 2's room and school age classroom, there was a metal floor transition strip that had 1/2" protrusions at each end leaving sharp metal edges. Children were observed napping on each side of this doorway within reach of the sharp edges.
2. An approximately 5' tall room divider was standing at the foot of a child napping in the 2's room. The divider was not secured and could fall on the child.

Description: Based on observation, facility did not ensure one staff member does not supervise more than one room or area during rest time.

Evidence: Staff #1 was observed supervising 4 children napping in the school age room (outside the restrooms) and 1 child in the 2's room. A doorway separated the two rooms/areas.

Description: Based on observation, facility did not ensure there is at least 12" of space between occupied cots, beds and rest mats.

Evidence: Four 2 year-old children were observed napping in the school age room outside the restrooms. Two of the occupied cots had 1" of space between them. Two of the cots had 3" of space between them.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview, facility did not ensure the diapering area has required items and soiled linens are stored in a leakproof covered receptacle.

Evidence: In the infant/toddler room, a child's soiled linens were stored in a plastic bag on the diapering table. Staff stated the bag was going to be placed in the child's diaper bag. Staff stated they did not have a covered receptacle for soiled linens.
