Comments:

An unannounced monitoring inspection was conducted on 8/10/2023 from 9:30am to 12:10pm. At the time of the inspection there were 52 children in care and 9 staff members. Children were observed participating in various activities and playing golf. Records were reviewed including six children?s records and six staff records. Posting requirements, medication and emergency supplies were reviewed during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Violations:

Standard #:	8VAC20-780-320-B
Description:	Based on observation and interview, it was determined the center did not ensure that each restroom area provided for children was equipped with disposable towels or an air dryer within reach of children. Evidence: 1. There was no paper towels or air dryer available in the boys bathroom. The paper towel dispenser was empty. 2. Staff #2 confirmed there were no paper towels in the dispenser.
Plan of Correction:	The paper towel dispenser was refilled.
Standard #:	8VAC20-780-510-G
Description:	Based on observation and interview, it was determined the center did not ensure that medication shall be labeled with the child?s name. Evidence: 1. One of two emergency medications for Child #6 (Benadryl) was not labeled with the child?s name. 2.The Director confirmed the child?s name was not on the box of medication
Plan of Correction:	The medication box was labeled with the child's name.
Standard #:	8VAC20-780-550-G
Description:	Based on a review of the emergency drill log and interview, it was determined that the center did not ensure that the program shall maintain a record of the dates of practice drills for one year. Evidence: 1.The emergency drill log did not contain documentation that an emergency drill was conducted for the month of July 2023. 2. Staff #1 (Director) stated that an emergency drill was conducted in July 2023 but it had not been documented.
Plan of Correction:	Staff will ensure that monthly practice drills are documented.
Standard #:	8VAC20-780-550-P
Description:	Based on record review and interview, it was determined the center did not ensure the record of children?s serious and minor injuries contained staff and parent signatures or two staff signatures. Evidence: 1. Five out of nine injury reports reviewed contained only one signature. 2. The Director confirmed there was only one signature on the injury reports.
Plan of Correction:	Staff will ensure that injury reports always contain the parent and staff signature's. If the parent does not sign the injury report, staff will ensure there are two staff members signature's and documentation on how the parent was notified.
