Comments:
An unannounced inspection was conducted on 8/17/2023 to investigate a complaint regarding diapering procedures. Based on evidence gathered through observations and interviews, the complaint was determined to be valid. A violation notice was issued to the facility. An exit interview was conducted on 8/18/2023.

Description: Based on observation, it was determined that not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a bottle of disinfectant solution sitting on an open shelf in the "Get Set" bathroom.

Description: (Repeat violation) Based on observation and staff interview, the facility failed to ensure that all diapering procedures, including using a nonabsorbent surface (changing table) for children under three and disposing of diapers in a leakproof receptacle that is foot-operated, are followed as required. Evidence:
1) There is no changing table in the "Get Set" classroom which includes children 24-30 months of age who wear diapers/pull ups.
2) The LI observed staff change a two year old child?s dirty diaper/pull up while the child was standing up in the bathroom.
3) The LI observed "Get Set" staff dispose of the dirty diaper/pull up using their hand to lift the trash can lid
4) Staff in the infant room stated and demonstrated that they use their hand to lift the trash can lid when disposing of diapers/wipes.
5) Diapers were observed in a swing top trashcan in the "first steps" classroom. Staff stated that they used this trashcan rather than the foot-operated trash can in the room.
6) In the preschool classroom, staff stated that one child wears a diaper, a couple children wear pullups all day, and others wear a pullup at naptime. There is no foot-operated trash can in the bathroom to dispose of these pullups.
7) Staff in the preschool classroom stated that pullups go into the trash can in the bathroom that does not have a lid and is not foot operated.
