Comments:
An unannounced monitoring inspection was conducted on 08/22/2023. There were 14 children, ages 2 years-4 years, 2 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during center time, going to large motor play in the gym, during story time and having a morning snack. A total of 5 children?s records, 3 staff records and 1 medication were reviewed. The inspector discussed school supply storage, medication, and annual training with the staff. The inspector arrived to the center at 8:55 am and departed at 10:50 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-280-E
Description: Based on observations, the center failed to ensure that cleaning materials and insecticides/pesticides shall be stored in areas physically separate from each other.

Evidence: There was an aerosol spray bottle of Hot Shot Flying Insect Spray stored in the same cabinet as various cleaning items such as Lysol and disinfectant spray.

Plan of Correction: The pesticide was removed from the classroom during the inspection.

Standard #: 8VAC20-780-510-G
Description: Based on observation and staff interviews, the center failed to ensure that medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: Staff 1 stated that Child 2 was administered Medication 1 each day at noon. Medication 1 was observed to be an oral solution that had been pre-drawn into an oral syringe. The oral syringe, containing Medication 1, was not labeled with the child?s name, the name of the medication, the dosage amount, or the times to be given.

Plan of Correction: The medication will be kept locked in the school office.

Standard #: 8VAC20-780-510-L
Description: Based on observation, the center failed to ensure that medication, except for those prescriptions designated otherwise by written physician?s order, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: Medication 1 was observed in an unlocked soft side insulated cooler/lunchbox. The cooler/lunchbox containing Medication 1 was observed sitting on top of the children?s cubbies. Medication 1 was not kept in a locked place that prevents access by children.

Plan of Correction: Will request that the medication be brought in the original, labeled container each day.

Standard #: 8VAC20-780-510-N
Description: Based on staff interviews, the center failed to ensure that centers shall keep a record of medication given to children.

Evidence: Staff 1 stated that they had administered Medication 1 a total of 6 times; once a day on August 14, 15, 16, 17, 18 and 21. Staff 1 stated that they had not documented the administration of Medication 1.

Plan of Correction: Will document the administration of the medication daily.
