Comments:
An unannounced renewal inspection was conducted on August 23, 2023 from approximately 9:20 AM until 11:35 AM. There were 84 children present with 17 staff supervising. The inspector observed the children watching a movie, playing outside, playing with toys, and handwashing. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, transportation, and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on interview and review of staff records, the center failed to ensure that all staff had a negative tuberculosis (TB) screening at the time of employment, no more than 30 days prior to hire, and before coming in contact with children.
Evidence:
1. Staff 3?s record contained a tuberculosis (TB) test that was not completed no more than 30 days prior to hire and before coming into contact with children. The tuberculosis (TB) test was dated 1/6/2023. Staff 3?s employment date was 3/23/2023.

Plan of Correction: While waiting for the background checks from another state, we will now have the out of state potential employees receive their tb test after the director?s receives all of their eligible checks. The assistant directors along with the director will notify the future staff members of receiving all the background checks in compliance with the law and regulations, then at this time the candidate will have to get a tb test to formally be an active staff member allowed to interact with the children at the center.

Description: Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
4. In the Preschool A classroom on the yellow wall, there was a piece of plexiglass that was cracked and split, creating a sharp edge that could cut children's skin.

Plan of Correction: During the inspection the plexiglass that was broken was removed. We have added the plexiglass to the daily classroom log sheet for the teachers to check and sign off on. If any of the plexiglass is damaged the teacher will let us know at the time of check and we will remove it asap. The directors will monitor all the plexiglass around the building and in classrooms for broken pieces or cracks. If there is any damage to any plexiglass, it will be removed asap.
