Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 08/24/2023, between the hours of 10 am to 1 pm. There were 113 children present, ranging from infant to preschool-age, with 30 staff supervising, across 10 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 10 children records, and 7 staff records were reviewed. In addition, 2 medications were also reviewed.

Information gathered during the inspection determined that there were (4) non-compliances with the applicable standards, and code. These violations were documented on the Violation Notice issued to the center.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review, 1 of 7 staff files reviewed indicated that the Child Day Center (CDC) did not obtain a copy of the results of a criminal history record (CRC) information check from any other state in which the individual has resided in the preceding five years before reporting to the child day center for work.

Evidence -
1. On the date of inspection (08/24/2023), the sworn statement completed by Staff #5 indicated that they had lived within the State of California (a non-NFF state) within the past 5 years.
2. While a Fingerprint Check was available from the Commonwealth of Virginia, a CRC (for Staff #5) was not available for review, nor was an email or other documentation from the State of California indicating that such information was not distributed outside of their state.
3. The start date at the CDC for Staff #5 was listed as 08/14/2023.

Plan of Correction: Documentation from the State of California for Staff #5 will be obtained and placed on file. Results of this finding will also be shared with the Licensing Inspector (LI).

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.

Evidence -
1. On the date of inspection (08/24/2023), cleaning supplies (in the form of two spray bottles) were observed (while not in use) on top of a storage cabinet, out in the open.
2. There were 13 preschoolers with 3 staff at the time of observation within Room #207.
3. While the cleaning supplies may have been high enough from the reach of the preschoolers in the room, the spray bottles were not kept in a locked place.

Plan of Correction: Corrected on the date of inspection.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, not all electrical outlets had protective covers.

Evidence -
1. On the date of inspection (08/24/2023), an outlet block in Room #202, and a power strip in Room #204, did not have protective covers on electrical outlets that were not in use.
2. Both rooms had preschool-age children in attendance; with these outlets in areas accessible to them.

Plan of Correction: Corrected on the date of inspection.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the diapering surface was no longer non-absorbent.

Evidence -
1. On the date of inspection (08/24/2023), within Room #101, a changing pad was observed on top of a changing table with a small rip in the upper left corner, exposing the padding from underneath.
2. With the padding now exposed, the changing pad is no longer considered to be non-absorbent.

Plan of Correction: Corrected on the date of inspection.
