An unannounced renewal inspection was conducted on 8/22/2023 from 10:40 am-12:15 pm. There were 49 children (infants-5yrs.) supervised by 10 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, and playing outside. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-780-160-A Description: Based on record review and interview, 1 of 5 staff files indicated that the facility has not ensured that each staff member has submitted documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment and prior to coming into contact with children.
Evidence:
1. The TB documentation on file for staff #4 was dated 06/25/2018 and 03/28/2019.
