A renewal inspection was initiated on 8/21/2023 and concluded on 8/21/2023. There were 38 children present with 11 staff supervising, two office staff and one cook. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and five staff records were reviewed. The inspection started at 9:35 am and concluded at 12:50 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: (REPEAT VIOLATION) Based on record reviews the center failed to ensure that each child shall have a physical examination by or under the direction of a physician: before the child's attendance or within 30 days after the first day of attendance.
Evidence:
Child #4, 6 and 7 do not have a physical examination on file within the set time frames.Plan of Correction: The director requested that these physicals be sent to the center either by the parent or the doctor's office.
Standard #: 8VAC20-780-160-A Description: (REPEAT VIOLATION) Based on record reviews the center failed to ensure that each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. This documentation shall be submitted at the time of employment and prior to coming into contact with children.
Evidence:
Staff #1 (hire date 6/20/23), staff #3 (hire date 6/6/23), and staff #4 (hire date 8/21/23) do not have documentation that a TB screening was submitted at time of employment and prior to coming into contact with children.Plan of Correction: The director will ensure that TB screenings are conduced within the set time frames and documented in the staff files.
Standard #: 8VAC20-780-70 Description: Based on record reviews the center failed to ensure that documentation of two or more references were checked before employment or volunteering.
Evidence:
Staff #1 (hire date 6/20/23) has one reference on file. Staff # 2 (hire date 8/1/23), staff #3 (hire date 6/6/23) and staff #4 (hire date 8/21/23) do not have documentation that two or more references were checked prior to employment.Plan of Correction: The director will ensure that references are checked and documentation is in the files.
