An unannounced mandated inspection was conducted on 8/29/2023 for the YMCA After School at Bedford Hills Elementary. There were 45 children, ages 5 -12 years and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies, and nutrition. Medications were observed during thew inspection. A total of 5 children?s records, 3 staff records and 3 medications were reviewed. The children were observed during arrival, self-directed activity, handwashing, snack time and homework time. The inspector arrived for the inspection at 3:17pm and departed the center at 4:23pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-350-B-5 Description: Based on observation, the center did not ensure that groups of school age children age 5-12 maintained a staff to child ratio of 1:18.
Evidence:
1. Licensing staff observed 26 children arrive by bus at approximately 3:42PM to join 19 children currently in attendance for a total of 45 children and only Staff 2 and Staff 3 were present. An administrative staff person arrived at approximately 3:49PM. Staff 2 and Staff 3 were out of ratio for approximately 7 minutes.Plan of Correction: Provider added third staff person to meet ratio.
