Comments:
An unannounced renewal inspection was conducted on 8/28/2023 from 11:20 am to 1:50 pm. There were seven children in care, ages three years to seven years, in two groups under the care of two staff. The Licensing Inspector reviewed three children?s records, three staff records, and other program records. The children were observed in indoor play, lunch, and reading (quiet time).

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on child record review, the center failed to obtain a physical exam form for a child within 30 days of beginning attendance.

EVIDENCE:
The record for child 3 has a documented start date of 8/22/2022 and the record does not have a physical exam.

Plan of Correction: The director will ask the parent to get a copy from the physician and give a copy to the center.

Standard #: 8VAC20-780-40-M
Description: Based on observation, interview, and child record review, the center failed to have a dated written list of all children?s allergies, sensitivities, and dietary restrictions.

EVIDENCE:
There was no posted list of children?s allergies, sensitivities, and dietary restrictions; this was confirmed by Staff 1. Review of children?s records show that Child 1 has a food sensitivity and Child 3 has medication allergies.

Plan of Correction: The director will review the children's records and create a dated list of allergies, intolerances, and dietary restrictions and keep it in each classroom.

Standard #: 8VAC20-780-510-L
Description: Based on observation, the center failed to have medications kept in a locked place.

EVIDENCE:
Tylenol and an antibiotic ointment were in the small first aid kit, which was on top of the cubbies and accessible to children.

Plan of Correction: These were removed from the first aid kit during the inspection.
