Discussed with the director that It is the operational responsibility of the licensee to ensure that the center's facilities are maintained in a clean, safe, and operable condition. Discussed the possibility of dead trees on the playground which may result in branches breaking off and could pose an unsafe environment for the children, staff, and others. It is advised that the trees on the school-age playground be evaluated to ensure they are in good health. It is important that staff are actively scanning the playground areas for any tripping hazards or unsafe conditions before children are allowed to go outside to play.

A complaint investigation concerning building maintenance was initiated on 8/23/2023, from a report received by the Fairfax County Licensing office on 8/21/2023. The investigation was conducted on 8/23/2023, in the morning between 10:15 am and 12:20 pm, and completed on 08/31/2023. The investigation included interviews with administration and staff, as well as an inspection of the physical plant. The preponderance of evidence does support the allegation and the complaint has been deemed valid. For violation details, please refer to the Violation Notice. Evidence: A medium-sized tree branch was found under a tree near the green tent, creating a possible tripping hazard on the school-age playground.Plan of Correction: Managers will continue to use the daily playground checklist to maintain a safe play space.
