An investigation in response into a self reported incident was initiated on June 7, 2023 and concluded on August 31, 2023. The self report received by the department included allegations in regards to forbidden action and behavior guidance. Staff were interviewed, documentation provided, and record was reviewed.
There was not preponderance evidence that center was not incomplaince. No violations.
