Comments:

An unannounced renewal inspection was conducted on August 22, 2023 from approximately 1:15 pm to 2:15 pm and on August 30, 2023 from 9:30 am to 11:00 am. There were no children present on August 22nd. On August 30th there were 21 children in attendance and a total of 7 staff present. 4 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, playgrounds, and bathrooms. The children were observed participating in a learning activity, singing songs, and eating snack, while the licensing inspector was on-site. Violations were cited as a result of this inspection. Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, date, and return it to me within 5 business days from today, 09/05/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description:	REPEAT VIOLATION Based on record review, and interview, the center failed to ensure each staff member shall submit documentation of a negative tuberculosis screening at least every two years from the date of the initial screening or testing. Evidence: The record for staff #4, date of employment 08/25/2020, contained documentation of a negative tuberculosis screening dated 11/12/2020. The screening was due on 11/12/2022. The Director confirmed that the record for staff #4 did not contain documentation of a repeat tuberculosis screening.

Description:	Based on record review, the center failed to ensure that staff complete the Virginia Department of Education (VDOE) sponsored orientation course within 90 calendar days of employment. Evidence: The record for staff #2, date of employment 05/08/2023, did not contain documentation of the VDOE course completion.

Evidence: 1. The record for staff #2, date of employment 05/08/2023, did not contain documentation of current certification in CPR and First Aid. Staff #2 was in classroom #2 with a group of children with staff #3, date of employment 08/17/2023. The record for staff #3 did not contain documentation of current certification in CPR and First Aid. 2. The record for staff #4, date of employment 08/25/2020, did not contain documentation of current certification in CPR and First Aid. Staff #4 was alone in the office with a child.
