Comments:
A monitoring inspection was initiated and concluded on 9/7/2023.There were 156 children present with 18 staff supervising and 3 administrative staff. The children were observed in their various classrooms engaged in centers, morning meeting time, art time, and reading time. A total of 10 child records and 6 staff records were reviewed. The inspection started at 9:00 am and concluded at 11:00 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, the center did not ensure to obtain documentation that each child has received a physical examination by or under the direction of a physician before the child's first day of attendance; or within 30 days after the first day of attendance.
Evidence: The record for child #1, (start date 2/22/2022) did not include documentation of a physical.

Plan of Correction: Child #1, had a physical completed and mom emailed the final page on 9/7/2023. *The child had the physical prior to starting at TPA.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review and staff interview the center did not ensure they had a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The director confirmed that Child #2 (start date 9/19/2022) did not have documentation of a written care plan that included the food to which the child has a confirmed food allergy.

Plan of Correction: Although we had a signed medication form from the child's doctor, we sent the form home for the doctor to write the plan to include the allergen.
