Comments:
An unannounced monitoring inspection was conducted on 09/11/2023 from 8:55am to 12:35pm. There were 136 children ages 3 months to age 4 years old supervised by 24 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 6 children?s records,11 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in math, literacy, snack, free play, reading, instruments, building and outside play. Diapering and handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT) and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation and staff interview, the center did not ensure they post the findings of the most recent licensing inspection of the facility.

Evidence: The most recent licensing inspection of the facility completed on 1/30/2023 was not posted as required. The posted inspection was dated 8/22/2022.

Plan of Correction: The most recent licensing inspection will be posted in the front lobby.

Standard #: 8VAC20-780-40-M
Description: Based on review of 6 child records, documentation and interview with the staff, the center did not maintain a current and dated written list of children?s allergies, sensitivities, and dietary restrictions that was accessible to all staff in each group or area where children are present.


Evidence: The record for Child #2 has documentation of a dietary restriction, but the dietary restriction is not documented on the list in the child?s Nursery 2 classroom.

Plan of Correction: All students allergies will be documented and provided to all teachers when a child enrolls or monthly as needed.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of 6 child records, the provider did not ensure they had a written care plan for each child with a diagnosed food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child #6 did not contain an allergy care plan for their physician diagnosed food allergy.

Plan of Correction: Will ensure that a written care plan is obtained for all children w/allergies.

Standard #: 8VAC20-780-80-A
Description: (Repeat Violation) Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.


Evidence: During inspection in the Preschool 2B classroom, there were 11 children present, but the written record reflected there were 9 children present.

Plan of Correction: Attendance record keeping procedures will be reviewed with all staff to ensure proper documentation is maintained.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, the center did not ensure that electrical outlets have protective covers.

Evidence: In the Threes classroom, there were 2 outlets on a surge protector that did not have protective covers.

Plan of Correction: All plugs will have protective covers when not in use.

Standard #: 8VAC20-780-330-B
Description: (Repeat Violation) Based on observation and inspection of the playground, the licensee did not to ensure that the required resilient surfacing surrounding playground equipment with moving parts or climbing apparatus created a sufficient fall zone.

1)The School Age playground has a platform at the climbing wall that is 96? inches in height with wood chips measuring at 0 inches in depth in front of the structure. The required depth of wood chips in this area should be 9 inches.
2) The Preschool playground has a slide that is 52? inches in height with wood chips measuring at 1 inch in depth on the right side of the structure. The required depth of wood chips in this area should be 6 inches.
3)The Toddler playground has a slide that is 18? inches in height with wood chips measuring at 1 inch in depth on the left side of the structure. The required depth of wood chips in this area should be 6 inches.
4)The Nursery 2 playground has a slide that is 40? inches in height with wood chips measuring at 1 inch in depth on the right side of the structure. The required depth of wood chips in this area should be 6 inches.

Plan of Correction: Mulch will be ordered again to put into place to meet required resilient surfacing on each playground under all climbing structures. Will use a playground safety checklist daily to monitor when levels become low.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center did not ensure that proper handwashing procedures were followed.

Evidence: In the Preschool 2B classroom, children did not wash their hands with soap and running water or disposable wipes before eating snack. Children were observed playing with toys and sitting on the floor immediately before being called over to the table to eat.

Plan of Correction: Handwashing procedures will be reviewed w/all staff and followed up by management.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure that disposable diapers were disposed in a storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence: In the Toddler classroom, the foot-operated storage system for disposal of diapers was not able to be operated by foot causing the staff member to have to touch the exterior surface of the trash can with their hand or a soiled diaper to open the trash can.

Plan of Correction: Will replace broken trash can wit the new one that was observed during inspection.

Standard #: 8VAC20-780-510-E
Description: (Repeat Violation) Based on review of 11 medications and medication authorizations, the center did not ensure they follow the policy for administration of medication.

Evidence:
1)Medication for Child #7 and Child #8 did not contain authorization from the child?s parent or from the physician.
2)In the Toddler classroom, Child #3 had a cup of milk containing medication that he is served each day in the classroom during snack time. The center does not have documentation of authorization from the child?s parent or physician to administer the medication.

Plan of Correction: Medication policies will be reviewed by all MAT cert employees. All authorizations will be obtained prior to accepting medicines.

Standard #: 8VAC20-780-510-I
Description: Based on review of 11 medications, the center did not ensure that they administer drugs that contained the prescription label identifying the child and the prescriber?s instructions pertaining to dosage, frequency and manner of administration.

Evidence:
1)The medication for Child #9 did not contain a prescription label with the child?s name, dosage information, frequency, and manner of administration as required.

Plan of Correction: Medications will not be accepted without prescription label.

Standard #: 8VAC20-780-520-C
Description: (Repeat Violation) Based on observation, the center did not ensure that diaper cream was kept inaccessible to children.

Evidence: In the Preschool 2A classroom, there were 2 diaper creams stored under the unlocked diaper changing station where children have access.

Plan of Correction: All cabinets that contain diaper cream will be locked at all times.

Standard #: 8VAC20-780-530-A
Description: Based on review of 5 staff records and staff interview, the center did not have at least one staff member in each classroom or area where children are present that had obtained first aid and cardiopulmonary resuscitation (CPR) certification.

Evidence: The file for Staff #1 did not contain documentation of CPR and First Aid certification. Staff #1 was alone in the Preschool 3A classroom so this classroom did not have at least one staff member that had documentation of cardiopulmonary resuscitation (CPR) certification in this classroom.

Plan of Correction: All new staff will obtain CPR/1st Aid and will not be alone in a classroom until certified.
