Comments:
A monitoring inspection was initiated on 9/15/23 and concluded on 9/15/23. There were 48 children present with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. The inspection started at 8:15am and concluded at 9:45am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on review of staff records the center failed to ensure that each staff has a completed Sworn Disclosure Statement prior to employment.

Evidence:
Staff #2, date of hire 8/21/23, did not have a Sworn Disclosure Statement as required.
This staff has been and was actively working with the children.

Plan of Correction: The Sworn Disclosure will immediately be completed and placed in the file. In the future all documents will be completed as required and placed in the file for review and compliance.

Standard #: 8VAC20-780-160-A
Description: Based on review of staff records the center failed to ensure that tuberculosis tests or screenings were completed no later than 30 days prior to hire.

Evidence:
The tuberculosis test/screening for staff #2 was dated 05/2023, the date of hire was 8/21/23. The test/screening was older than the allowed 30 days.

Plan of Correction: A new TB screening/test will be completed and placed in t he file. In the future, TB tests/screenings will not be over 30 days old as of the hire date.

Standard #: 8VAC20-780-70
Description: Based on review of staff records and discussion with administration, the center failed to ensure that required staff information was on file.

Evidence:
1. Staff 2, hire date 8/21/23, did not have an emergency contact, health problems, or job title documented as required.
2. Staff 4, hire date 8/12/23, did not have an application, emergency contact, health problems, or job title documented as required.

Plan of Correction: All missing items will be completed and placed in the staff file as required. In the future, all required items will be completed in the allowed time-frames.

Standard #: 8VAC20-780-240-B
Description: Based on staff record review the center failed to ensure that staff receive orientation training as required.

Evidence:
1. Staff #2, date of hire 8/21/23, did not have proof of completed orientation training as required.
2. Staff #3, date of hire 9/11/23, did not have proof of completed orientation training as required.
Both staff were and have been actively working with the children according to staff schedule, interview with administration, and this inspection.

Plan of Correction: Orientation will be completed and documented in the file as required. In the future, all items required to be completed will be and also documented in each file.
