Technical Assistance:
Consultation was given on injury documentation, allergy care plans and model forms on the VA DOE Licensing website: childcareva.com

Comments:
An unannounced renewal study was conducted in the newly renovated space. 6 children were present with 2 staff and were on the playground upon arrival. Observed transition into the classroom, hand washing and play time in the large open space. The program now offers before and after care as well as the dedicated preschool program. Additional staff are available for the extended day portion of the afternoon. Reviewed physical space, records for children and staff, emergency preparation, trainings and medication. Lunches are bought from home. The staff records and administrative records are kept at a central location with the Parks and Recreation staff. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-90--A
Description: Based on record review, 3 of 3 children's records were missing a parent agreement between the center and the parent.
Evidence:
Child #1-3 did not have the statement in their record that the parent will notify the center within 24 hours if the child or any member of the immediate household has developed any reportable communicable disease as defined by the State Board of Health.

Plan of Correction: Information is on file for each child. Admin staff will review access with program staff.

Standard #: 8VAC20-780-550-P
Description: Based on record review, 2 injury reports were incomplete.
Evidence:
Both injury reports did not include "any future action to prevent reoccurrence"

Plan of Correction: Staff will update reports and include this information in all future reports.
