Comments:
An unannounced renewal inspection was conducted on 9/7/23 from 6:25am to 7:05am with program staff. There were five children in care, supervised by two staff. The manager was on-site for part of the inspection as well. Children were observed having free play. Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. Two child records were reviewed. A review of required on-site documentation was conducted for staff certified in CPR/First Aid, Daily Health Observation Training, and Medication Administration Training (MAT). Off-site staff records were reviewed at the YMCA office on 9/7/23. One medication and its authorization were reviewed and the center has three staff with current certification in MAT. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/25/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-160-A
Description: Based on review of four staff records, the program did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: The TB test on record for Staff #4, with an employment date of 8/11/22, was dated 8/13/22.

Plan of Correction: Ensure all staff have submitted all TB paperwork before hired.
