Comments:
An unannounced inspection was initiated on 9/15/2023 and concluded on 9/21/2023 in response to a complaint that was received by the department on 9/12/2023 regarding allegations in the areas of injury reporting, supervision, building maintenance, and toileting procedures. The inspection was initiated on site on 9/15/2023. The inspector was on site from approximately 1:47-2:52 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted remotely.

The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. In addition to the original complaint, there were also violations found during the inspection regarding CPR and first aid requirements for staff. See the violation notice for more details.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: COMPLAINT RELATED

Based on interview, the center did not ensure that children younger than three years were changed on a changing table or countertop designated for changing as required.

Evidence:
1. During interview, staff in a two-year old classroom stated that children were sometimes diapered while standing in the bathroom as part of their potty-training procedure.
2. Administration acknowledged that the children in the two-year old classroom were not always being changed on the designated table.

Plan of Correction: The director retrained staff members on proper diaper changing procedures and will continue to monitor classrooms so that all procedures are being followed.

Description: Based on record review and interview, the center did not ensure that at least one staff in each classroom or area where children are present had current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care as required.

Evidence:
1. Staff #1 stated that she did not have current certification in CPR but did occasionally supervise the two-year old classroom independently.
2. Administration acknowledged that a CPR certified individual was not present in the two-year old classroom during the times when staff #1 worked alone.

Plan of Correction: Staff member #1 has already been enrolled in a CPR class.

Staff members will be enrolled in a CPR class on their first day of employment.
