The inspector conducted an unannounced renewal inspection from 10;10 a.m. until 1:30 p.m. There were 154 infants and children in care with staff supervising at all times. The cook and administrative staff were also present, and an art teacher. infants were observed sleeping, being
fed, and playing with musical toys. Children were observed going to the art room, washing hands, being read to, eating lunch, being checked out by parents, being loaded on to a van, and during rest time. Ten staff records and 11 children's records were reviewed. Medications and the
corresponding paperwork were reviewed. Violations are identified on the violation notice. Thank you for your cooperation. Please all your inspector at 276-698-9981 with any questions.

Standard #: 8VAC20-780-270-A Description: Based on observation, the center did not maintain some equipment at the center in an
operable condition.
evidence:
1. The inspector observed 4 loose foot holds on the climbing wall on the deck area, and
lattice that had splintered near the bottom of the deck.Plan of Correction: The foot holds and lattice area will be
fixed.
Standard #: 8VAC20-780-330-B Description: Based on observation, the center did not maintain the minimum requirement for
mulch/resilient surfacing on the playground.
Evidence:
1. The inspector measured mulch around the green slide and at the bottom it was 2-3
inches.
2. The blue and purple spinners had 2-3 inches of mulch underneath them.
3. The climbing wall had 2-3 inches under it.Plan of Correction: Mulch will be added and redistributed to the areas that need it.
