An unannounced renewal inspection was conducted on September 25 2023 between 10:30am and 11:45am. There were 61 children in the direct care of 4 staff members. Children were observe participating in a tennis tournament, playing wall ball and kickball, washing hands, and playing kickball. A sampling of child records, staffing records, emergency drills for the past 12 months, the menu, flashlight, and the physical space was reviewed and inspected. Standard #: 8VAC20-780-130-A Description: Based on record review, the center did not ensure that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
Child #1 record does not have documentation of an immunization record.Plan of Correction: Staff will get the documentation from the school office.
Standard #: 8VAC20-780-140-A Description: Based on record review, the center did not ensure that each child has a physical examination under the direction of a physician prior to attendance or within 30 days of their start date.
Evidence:
Child #1 record did not contain a documented physical. The child began care on August 23 2023.Plan of Correction: Staff will get the documentation from the school office.
