Technical Assistance:

Discussion of using the online portal for initial CPS checks versus using it for portability. Discussion of Maryland background checks that have been sent.

Comments:

This monitoring inspection was initiated by licensing staff and concluded on 09/21/2023, between the hours of 9:30 am to 12:30 pm. There were 19 children present , ranging from infant to preschool-age, with 6 staff supervising, across 4 rooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 7 staff records were reviewed. Information gathered during the inspection determined that there were (5) non-compliances with the applicable standards, and codes. Standard #:	22.1-289.035-B-2
Description:	Based on review, 1 of 7 staff files indicated that not all fingerprint checks were conducted and received prior to the start date of each staff person. Evidence - 1. On the date of inspection (09/21/2023), a fingerprint check was not available for Staff #5 (start date - 08/07/2023). 2. The center was currently waiting on receiving these results.
Plan of Correction:	Center is waiting on results.
Standard #:	22.1-289.036-B-2
Description:	Based on review, 2 of 7 staff files indicated that fingerprint checks observed on site were older than 5 years; expired. Evidence - 1. On the date of inspection (09/21/2023), the following staff had expired fingerprint checks on file: Staff #1 (dated - 07/05/2018), and Staff #6 (dated - 04/16/2018). 2. This was confirmed on the date of inspection by center admin.
Plan of Correction:	An appointment will be scheduled for these two staff in order to obtain these results.
Standard #:	8VAC20-780-160-A
Description:	Based on review, 1 of 7 staff files reviewed indicated that each staff member did not submit documentation of a negative tuberculosis (TB) screening (not older than 30 days) prior to their start date. Evidence - 1. On the date of inspection (09/21/2023), TB documentation was not available for review for Staff #5. 2. The start date for Staff #5 was listed as 08/07/2023.
Plan of Correction:	Staff will obtain updated TB documentation.
Standard #:	8VAC20-780-60-A
Description:	Based on review, 1 of 5 children files indicated that not all of the required documentation was contained within each child's record. Evidence - 1. On the date of inspection (09/21/2023), the health forms (physical and immunization page) was not available for Child #5. 2. The start date for Child #5 was listed as 01/03/2023.
Plan of Correction:	Health forms will be obtained and placed on file.
Standard #:	8VAC20-780-240-B
Description:	Based on review, 1 of 7 staff files indicated that a staff person did not complete an orientation training (in subsection C of this section) prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Evidence - 1. On the date of inspection (09/21/2023), an orientation training was not available for review for Staff #5. 2. The start date for Staff #5 was listed as 08/07/2023. Staff #5 was onsite today during today's inspection.
