Comments:
An unannounced renewal visit and facility tour was conducted on 09/20/2023. At the time of inspectors' arrival there were 45 school aged children in care with 4 staff members. Children were observed interacting with staff, engaging in homework, snack, handwashing, and departure. A sample of 5 children's records and 4 staff records were reviewed. Injury reports were also reviewed. Staff reported medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review and interview it was determined the center did not ensure each staff member does not have a central registry finding within 30 days of employment or volunteer service.
Evidence:
1. The staff record for staff #4(hired 6/5/23) did not contain documentation of a central registry search.
2. Staff confirmed this documentation was not available during this inspection.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview it was determined the center did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
Evidence:
1. The records for child #6 and #7 stated that they had food allergies, there was no allergy list posted.
2. Staff conformed that there was no allergy list available during this inspection.

Description: Based on record review and interview it was determined the center did not ensure the record for each child contained all of the required information.
Evidence:
1. Five of the five records reviewed did not contain documentation of any previous child day care and schools attended by the child.
2. Five of the five records reviewed did not contain documentation of proof of the child's identity and age.
3. Staff confirmed that these records did not contain this documentation.

Description: Based on record review, the center failed to ensure that staff records contain all of the required elements.

1. Staff #3 is listed as a program leader in her employment file however, there is no written documentation to demonstrate that she possess the education, certification and experience required by the program leader job position.
2. Staff confirmed that this documentation is not available.

Description: Based on observation and interview it was determined the center did not ensure that the restroom area provided for children contained all of the required components.
Evidence:
1. The boys restroom did not contain paper towels, subsequently, there was water dripped along the floor.
2. Staff confirmed that there were no paper towels available at this time.
