An unannounced inspection was conducted on 9/13/2023 to investigate a complaint regarding operational responsibilities. Based on evidence gathered through observations, interviews, and documentation review the complaint was determined to be valid. A violation notice was issued to the facility. An exit interview was conducted on 9/26/2023.

Standard #: 8VAC20-780-40-E Complaint related: Yes Description: Based on observation, interviews and a review of documentation, it was determined that the center's activities did not comply with the terms of the current license. Evidence:
1) At approximately 4:15pm, the whiteboard listed 64 children on the three to six year old playground and 24 children on the toddler playground. Staff interviewed stated that the whiteboard is used to identify the number of children present on the playground.
2) At approximately 4:15pm, the LI observed 62 children playing on the three to six year old playground and 22 children playing on the toddler playground.
3) The stipulation on the license states that only 50 children can be on the three to six year old playground and only 19 children on the toddler playground at any one time.Plan of Correction: Not available online. Contact Inspector for more information.
