Comments:
An unannounced complaint inspection was initiated on 09/22/2023 from 9:55am to 12:05pm in response to a complaint regarding neglect and injury reporting. Interviews were conducted with the staff, documentation was reviewed, videos were observed and the playground was observed. Based on the preponderance of evidence collected during the investigation, the complaint was determined to be not valid. There were 2 violations discovered during the investigation that were unrelated to the complaint. The exit interview was conducted on 9/29/23 with the Director. Areas of non-compliance are identified in the Violation Notice.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 10/6/23. Please email me at angela.dudek@doe.virginia.gov with any questions.
