Comments:
An unannounced monitoring inspection was conducted while on a Conditional license on September 29, 2023, from 9:50am-10:40am. At the time of entrance, ( 2) children were in care with ( 1) staff members present. The inspector was told the owner was out of the country. The owner was called and told the Inspector that the records for the staff and children were not accessible. Staff stated that she had the parents phone numbers on her private phone. The 2 children and 1 staff was observed in classroom # 108., another child was to arrive shortly. The other 2 classrooms and office were observed. Children were engaged in manipulatives and learning shapes. The 2 children lunch boxes were dated and labeled, lunch would be catered into the CDC. Several violations were cited, many with higher risk due to the fact that there is only 1 staff on the premises at all times this week and next week. Areas of non-compliance are identified on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

The facility is reminded that during the 6 month Conditional license, it is the time to show that they can comply with the licensing standards.

Standard #: 22.1-289.011-F
Description: Based on observation and staff interview, the license and most current inspection summary are not posted in a conspicuous place on the licensed premises.
Evidence: The staff and children were in room # 108. The staff stated that the children are dropped off at this room. There was no posting of the license or most recent inspection summary in room # 108 or anywhere else in the licensed area of the child care center.

Description: Based on observation and staff and owner phone interview, the records for staff and children were not made accessible.
Evidence: The Inspector arrived at the child day care center at 9:50 am. There was 1 staff with 2 children. The staff got the owner on the phone, she stated she was out of the country and could email the records to me. The owner said the staff did not have access to the records. The records, to include medical emergency authorization, parent phone numbers and emergency phone numbers for the 2 children enrolled was not accessible.

Description: Based on observation and staff interview, the center does not have a qualified program director regularly on site at least 50% of the centers hours of operation.
Evidence: The owner was not on site today, nor this week, she is out of the country. Staff stated she will return after the Columbus holiday (October 9th).

Plan of Correction: Program director on premises at least 50 % of the centers hours of operation.

Description: Based on observation and staff/owner interviews, there is no one staff on duty who has obtained the daily health observation of children.
Evidence: There was only 1 staff on site today, the staff's record was not accessible to determine if they have daily health observation training.

Standard #: 8VAC20-780-340-C
Description: Based on observation and staff and owner phone interview, there was no way to determine if the one staff on the premises meets the qualifications of program lead or director. And there was no other staff available.
Evidence: There was only 1 staff on the premises.,because the staff record was not accessible there was no way to determine if the one staff met the qualifications of a program lead or program director. There was no other staff t that was immediately available. The staff said the owner has been gone this week and the following week. The staff was caring for 2 children by herself.

Standard #: 8VAC20-780-530-C
Description: Based on observation and staff and owner phone interview there was not at least 2 staff members who meet the requirements of current CPR and first aid training on the premises during the center's hours of operation and where children are in care.
Evidence: The inspector arrived at 9:50 am, there was 1 staff onsite with 2 children in care and another to arrive later. The records to determine if the one staff was first aid and CPR certified were not accessible. The owner was called and said she was out of the country. Staff said the children were in care last week and all this week. The owner was not present this week and will be returning after the Columbus Day holiday (October 9th).
