Technical Assistance:
Today we discussed the requirement that the results of the most recent inspection are posted visibly in the center. We discussed specifically, the requirement that the violation Notice (if one is issued) is posted without other pages of the report covering it and that it is at a level and location where parents can comfortably and easily read it, if they choose to.

Comments:
A monitoring inspection was initiated and concluded on 9/25/2023. There were 34 children, ages 2 to five years old, present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing in centers and with table top activities, eating lunch and settling onto cots for rest time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 10:30 am and concluded at 12:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Repeat violations were found and are identified within this report.

Violations:
Standard #: 8VAC20-780-70
Description: ***REPEAT VIOLATION***
Based on a review of records and interview, the licensee did not always keep all of the required records for each staff person.
Evidence:
Five staff records were reviewed.
The record for staff #1(employed 7/25/2023) contained documentation for only one of the two required references as to character and reputation as well as competency having been checked before employment.
The record for staff #1 did not contain the name, address, and telephone number of a person to be notified in an emergency which must be kept at the center.

Plan of Correction: The program director will ensure that two references for new staff are checked and documented in the record prior to employing them.
A second reference will be obtained for staff #1 and placed in the record.
The program director will ensure that the name, address, and telephone number of a person to be notified in an emergency is documented in each staff record.

Standard #: 8VAC20-780-420-E-3
Description: Based on a review of records and interview, the licensee did not ensure that staff request, at least annually, documentation that the required information in the child's record is up to date.
Evidence:
Five children's records were reviewed.
The record for child #2, whose enrollment date was documented as 12/1/2020, did not contain documentation, that the parent had confirmed, since enrollment, that the required information in the child's record is up to date.
The program director confirmed that there was not a review / update to the record documented.

Plan of Correction: An update will be requested obtained for child #2 and a schedule for all children's records to be reviewed annually and updated, as needed will be put into place. Records will contain documentation of the annual review and any updates.
