Comments:
An unannounced monitoring inspection was completed from 12:20pm until 1:15pm based on a self reported incident that occurred 09/12/23. Staff interviews were conducted and records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on interviews and record review, the center failed to ensure when staff are supervising children, they always ensure their care, protection, and guidance.
Evidence:
1. Staff #1 and staff #2 stated child #1, two years, eight months old, was inside the classroom alone and unsupervised for approximately three (3) minutes.

Plan of Correction: Procured a doorbell alarm system for all exterior classroom doors to alarm and alert when opened. Completed: 9/15/23
? Procured and adjusted all exterior classroom doors to have slow catch hinges that have been installed and adjusted to prevent doors from closing too quickly. Completed 9/19/23 ? As of 9/13 ? 9/29/23 all staff will be retrained on Child Supervision Record expectations and use of tool. ? As of 9/13 ? 9/29/23 staff will be retrained on our supervision policy and expectations.

Description: Based on interviews and record review, the center failed to ensure that children under 10 years of age are always within actual sight and sound supervision.
Evidence:
1. Interviews with staff #1 and staff #2 determined that child #1, two years, eight months old, was left out of sight and sound supervision of staff for an undetermined time. Staff #1 stated that she went to the fenced playground at 9:30am with her class. There is a date stamped picture of child #1 on the playground at 10:03am. During a head count staff #1 noticed child #1 was no longer on the playground. Staff #1 went to classroom which is attached to the playground and saw child #1 in the classroom. Staff #2 stated that she was in the hallway and noticed child #1 in the classroom alone at 10:33 am and this was the time staff #1 was opening the door to look for the child. Staff #1 states the approximate time the child was alone in the classroom was three (3) minutes.
