A monitoring inspection was initiated on September 29, 2023 from 8:05am - 10:00am. There were 29 children with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-A Description: Based on record review, the licensee did not ensure the center obtain documentation that each child has recieved immunization before child can attend the center.
Evidence: The center did not obtain Child #1 and Child #2 immunization record prior to attending the program.Plan of Correction: All children files will be updated to include physicals.
Standard #: 8VAC20-780-140-B Description: Based on record review, the licensee did not ensure each child have a physical examination prior to attendance or with 30days from start date.
Evidence:
Child # 2 start date 08/28/2023. No physical obtained by the center.Plan of Correction: All children files will be updated to include immunization forms.
