A monitoring inspection was initiated on September 29, 2023 from 9:50am - 11:35am. There were fourty children present with 9 staff supervising. Observed diapering, children singing, sharing objects that were brought from home and station activities. A total of four children records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the .program

Standard #: 22.1-289.035-A Description: Based on record review and staff interview, the licensee did not ensure all employees background check is completed every 5 years.
Evidence:
Staff #1 most recent fingerprint background check was 04/20/2018.
Staff #2 most recent fingerprint background check was 08/21/2018.
Both background checks were more than 5 years old.Plan of Correction: Employees updated background check on 10/02/2023. Received both on 10/04/2023.
Standard #: 22.1-289.035-B-4 Description: Based on record review and staff interview, the licensee did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
Staff #3 (hire 04-2023) did not have out of state child abuse and neglect registry for Pennsylvania. Confirmed by Director.Plan of Correction: Employee requested check on 10/02/2023.
The check was recieved on 10/04/2023.
Standard #: 8VAC20-780-160-B Description: Based on record review, the licensee did not ensure at least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.
Evidence:
Staff #2 most recent tuberculosis was 08/11/2021. The tuberculosis test was more than 2 years old.Plan of Correction: Emplouee went and had new screening 09/29/2023.
Standard #: 8VAC20-780-560-F Description: Based on record review, the licensee did not ensure a menu listing foods to be served for meals and snacks during the current one-week period was dated.
Evidence:
The menu posted was not dated. Confirmed by Director.Plan of Correction: Fixed the menu to reflect current week's date.
