An unannounced non mandated inspection was conducted on 10/02/2023. There were 15 children, ages 5 years-12 years, and 2 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, and snack. A total of 2 staff records, were reviewed. The inspector arrived at 3:20 PM and departed at 4:00 PM.

Standard #: 22.1-289-036-B-4 Description: Based on review of one board officer?s the center failed to ensure that all out of state
background checks were obtained.
Evidence:
Board Officer # 1 took office on 07/17/2023 and lived outside of the state of Virginia in the past
five years in a NFF state (national fingerprint file). The Board Officer is required to

Based on observation, staff record review, and interviews, the center failed to ensure that each grouping of children had a qualified program lead.
Evidence:
1.Staff # 1 was observed working as a program leader on the day of the inspection. Staff # 3 stated that he was the program lead. After review of Staff # 1 record, there was no documentation of the educational requirements for a program leader position.Plan of Correction: Not available online.
