Comments:
An unannounced renewal inspection was conducted on 10/3/23 from 7:30am-8:00am with program staff. There were three children in care, supervised by two staff. Children were observed doing a Halloween craft activity. Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. Two on-site child records were reviewed. A review of required on-site documentation was conducted for staff certified in CPR/First Aid, Daily Health Observation Training, and Medication Administration Training (MAT). An off-site staff record review was also conducted on 10/3/23 from 2:00pm-3:15pm at the Ron Rosner YMCA office. Board Officer background checks were also reviewed. Staff reported that there were no medications on-site but the center has staff with current certification in Medication Administration Training (MAT). Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 10/20/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.036-A
Description: Based on review of five Board Officer/Agent records, the program did not obtain the results of a national, fingerprint-based criminal history record check every five years from the date of the last check for each Board Officer. Evidence: The most recent fingerprints results on record for Board Officer A were dated 9/21/18.

Plan of Correction: We will continue to communicate with Corporate board to ensure requirements are being completed.

Standard #: 8VAC20-780-160-A
Description: Based on review of two staff records, the program did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: The record for Staff #2, with an employment date of 5/18/23, did not contain documentation of a TB test or screening.

Plan of Correction: Staff will completed and provide TB screening results prior to working on the floor.

Standard #: 8VAC20-780-70
Description: Based on review of two staff records, the program did not obtain all required documentation for staff records. Evidence: 1. The record for Staff #2 did not contain documentation of the program obtaining two references as to character and reputation before employment for the staff member.

Plan of Correction: References will be completed prior to hire.

Standard #: 8VAC20-780-240-A
Description: Based on review of two staff records, the program did not ensure that the Virginia Department of Education-sponsored orientation course (Preservice) was completed within 90 calendar days of employment by all staff. Evidence: The record for Staff #2, with an employment date of 5/18/23, did not contain documentation of the Preservice certificate.

Plan of Correction: Ensure staff are completing trainings in the necessary timeframe.
