Comments:
An unannounced monitoring inspection was conducted on 10/5/23 from 9:10am to 12:00pm with both Directors. There were 83 children in care at the beginning of the inspection, ranging in age from seven-weeks to four-years-old, supervised by 15 staff; more children arrived throughout the inspection. Infants were observed being fed bottles, having their diapers changed, and playing on the floor with toys. Older children were observed looking at books, having diapers changed and washing their hands, eating a snack of bananas and milk, talking about emotions and colors while using flashcards, playing with play food, listening to the teacher read a story, transitioning to ?Soccer Shots?, and playing in structured centers. Six child records and five staff records were reviewed. The Center has 29 staff with current certification in CPR and First Aid, and two staff trained in Daily Health Observation. Two medications and authorization forms were reviewed and the center has 14 staff with current certification in Medication Administration Training (MAT). Required postings were observed. The attendance, menus, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 11/14/22 and the most recent Health Inspection on file was dated 3/28/23. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 10/13/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on review of six child records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: The record for Child D, with a first day of attendance of 10/2/23, did not contain documentation of immunizations.

Plan of Correction: The School has a checklist procedure in place to ensure all required documentation in provided to the school prior to a child's first day of attendance. The School followed up with the Parent of Child D to obtain the previously requested immunization documentation. The Parent subsequently provided the required documentation on 10/9/2023.

Standard #: 8VAC20-780-70
Description: Based on review of five staff records, the center did not obtain all of the required documentation for staff records. Evidence: The records for Staff #1, with an employment date of 9/25/23, and Staff #5, with an employment date of 7/5/23, did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Plan of Correction: The School has a checklist procedure in place to ensure all required background and reference checks are obtained prior to a staff member's first day of employment. The School followed up with the references for Staff #1 and Staff #2, and received affirmation of character, reputation and competency of Staff #1 on 10/5/23, and Staff #2 on 10/6/23.

Standard #: 8VAC20-780-510-L
Description: Based on observation, the center did not ensure that medications, except for those prescriptions designated otherwise by written physician's order, were kept in a locked place using a safe locking method that prevented access by children. Evidence: 1. On 10/5/23, in the ?PreK1? class for three- and four-year-olds, a child?s emergency medication was observed on a high shelf in an unlocked cabinet. 2. On 10/5/23, in the ?PreK2? class for three- and four-year-olds, a child?s emergency inhaler was observed on a high shelf in an unlocked cabinet.
