A subsidy health and safety (SHSI) inspection was completed on 8/23/2023. A Voluntary Registered Monitoring inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Standard #: 22.1-289.035-A Description: Based on a review of records and interview, the caregiver did not undergo a criminal background check within 5 years of the most recent documented background check results.
Evidence:
The most recently dated criminal record check for caregiver #1 was dated 5/16/2018 which is more than 5 years ago.
The provider confirmed that she had not obtained an updated Criminal Record Search since 5/16/2018.Plan of Correction: The provider will request an appointment within 10 days of the inspection to have fingerprints retaken in order to complete an updated criminal record search.
The provider will send written documentation showing confirmation of the appointment within 10 days and will forward the results to the inspector upon receipt.
Update - the results of the Criminal Record Check were received by the licensing inspector on 9/1/2023.
Standard #: 8VAC20-790-250-F Description: Based on a review of records and interview, the provider did not annually attend at least 16 hours of training, to include the department's health and safety update course.
Evidence:
The provider was unable to locate documentation of the department's health and safety update course. The original training was present in the record and was dated 1/21/2017.
The provider confirmed she was unable to locate an annual update training certificate during the inspection.Plan of Correction: The provider requested and received the links via email for the required training on 8/24/2023.
The Provider will begin the training with the goal of completing it within 3 weeks.
Standard #: 8VAC20-790-410-B Description: ***REPEAT VIOLATION***
Based on observation and interview, the provider does not have all of the required First Aid and Emergency Supplies available.
Evidence:
The provider confirmed that she does not have a battery operated radio in her home.Plan of Correction: The provider stated that she would purchase a battery operated radio and the appropriate batteries within 30 days.
