Comments:
An unannounced renewal inspection was conducted today from 9:00 am-11:15 am. There were 69 children (infants-5yrs.) directly supervised by 12 staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, circle time activities, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Description: Based upon record review, required information for child record was incomplete.
Evidence:
Child A's record was missing addresses for emergency contact #1 and #2.

Plan of Correction: I will get the missing information today at pick up from mom.

Standard #: 8VAC20-780-70
Description: Based upon record review, required documentation was not complete.
Evidence:
Staff A's record is missing documentation of 2 or more references as to character and reputation as well as competency were checked before employment.

Plan of Correction: I spoke with the references. I will call again to document for the file.

Standard #: 8VAC20-780-90--A
Description: Based on record review, written agreement between the parent and center were incomplete.
Evidence:
Child A's record was missing a signed authorization for emergency medical care on file by the first day of attendance.

Plan of Correction: I will have mom sign today at pick up.

Standard #: 8VAC20-780-500-B
Description: Based on observation, diaper changing pad was no longer considered to be nonabsorbent.
Evidence:
There was a tear in the nonabsorbent material covering the diaper changing pad.

Plan of Correction: I will order a new pad today.

Standard #: 8VAC20-780-540-C
Description: Based on observation, required components of the first aid kit were not available.
Evidence:
The first aid kit did not contain more than 1 triangular bandage. 0 were found in the first aid kit.

Plan of Correction: Item was ordered during inspection.
