Comments:
An unannounced renewal study was conducted today after school.
There were 45 children present with 3 staff.
3 staff records were reviewed for specific items only. The full staff record is reviewed at another time at the main office. Additional documentation was received regarding staff qualifications.
4 child records were reviewed. All were complete.
Children were observed transitioning to the program, playing on the playground and in the cafeteria.
Emergency supplies, first aid and CPR certifications, MAT certification, emergency evacuation drills were all in compliance.
The van used to transport children was also inspected today.

Standard #: 8VAC20-780-190-A
Description: Based on a review of documentation provided, the program director does not meet the qualifications for the position.
Evidence:
Staff A's record does not include documentation of a degree in child related field, or 48 credit hours with 12 in a concentrated area related to children.

Plan of Correction: I will research our documentation to verify qualifications.

Standard #: 8VAC20-780-550-K
Description: Based on a review of documentation, the center did not prepare a document containing local emergency information, potential shelters, hospitals, etc., that pertain to each site frequently visited or routes frequently driven.
Evidence:
There is no document available in the notebook on the van that shows: frequently traveled routes, potential shelters, hospitals, etc.

Plan of Correction: The van's book will be updated to include all required information.

Standard #: 8VAC20-780-580-B
Description: Based on observation, the vehicle used to transport children, does not meet the standards set by the Department of Motor vehicles and kept in satisfactory condition to assure the safety of children.
Evidence:
The van used to transport children had an expired sticker on the license plate. The sticker was dated August 2023.

Plan of Correction: I have contacted the Administrative Director for Transportation for our agency.
They will be getting the tags renewed as soon as possible.
