Technical Assistance:
Provided consultation on standard:
1. 80: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The attendance record helps first responders, and others in an emergency, easily identify who is and is not in the classroom at any given time.
2. 240I: documentation of orientation training shall be kept by the center in a manner that allows for identification by the individual staff member and shall include:
a. Name of staff
b. Training topics
c. Training delivery method
d. The entity or individual providing training
e. The date of the training

Comments:
An unannounced monitoring inspection was conducted from 10:30am - 2:30 pm with the center director. There were 102 children, ranging from two months to four years of age, present with 17 staff supervising. Infant care was observed, and older children were observed playing parachute games, engaged in sensory activities, completing seasonal arts and crafts projects, and participating in circle time activities. Five child records and six staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, 15 medications and authorization forms, and the fire and health inspections.

Standard #: 22.1-289.035-A
Description: (Systemic) Based on review of six staff records, it was determined that not all staff had a repeat Central Registry Search every five years as required. Evidence: The record for staff #6 had a Central Registry search results letter dated: 12/19/2017.

Plan of Correction: Central Registry was sent in 10/16/2023.

Standard #: 22.1-289.035-B-2
Description: (Systemic) Based on review of six staff records, it was determined that not all staff had an Office of Background Investigations (OBI) fingerprinting-based criminal history record check prior to their first day of employment. Evidence: the record for staff #3 (date of rehire: 5/25/2023) had a fingerprint determination letter dated: 8/19/2019 and staff #5 (date of hire: 5/1/2023) had a fingerprint determination letter dated: 5/10/2023.

Plan of Correction: Moving forward all staff will have fingerprints done before their start date.

Standard #: 22.1-289.035-B-4
Description: (Systemic) Based on review of six staff records, it was determined that Out of State background checks (Sex Offender Registry search and Child Abuse and Neglect search) were not conducted as required for staff who indicated they have lived outside of Virginia in the past five years. Evidence: the record for staff #1, (Date of hire: 4/24/2023), who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state sex offender registry search or an out of state child abuse and neglect search request.

Plan of Correction: Moving forward making sure all staff who have lived out of state will have the proper out of state checks done. Staff #1 background check in progress.

Standard #: 8VAC20-770-60-C-2
Description: (Systemic) Based on review of six staff records, it was determined that not all staff had Central Registry Search results within thirty days of employment. Evidence: The record for staff #1 (Date of hire: 4/24/2023), staff #3 (Date of hire: 5/25/2023) and staff #5 (Date of hire: 5/1/2023) did not have documentation of a Central Registry search request.

Plan of Correction: Files were skimmed through and all central registries that were needed to be send in will be by 10/20/2023.

Standard #: 8VAC20-780-160-A
Description: Based on review of six staff records, the facility did not have documentation of a negative tuberculosis (TB) test/ screening for all staff at the time of employment that was completed within the last 30 calendar days of the date of employment. Evidence: the record for staff #1 (date of hire: 4/24/2023) had a TB test/screening dated: 11/29/2022. The record for staff #3 (date of rehire: 5/25/2023) had a TB test/screening dated: 10/5/2021. The record for staff #5 (date of hire: 5/1/2023) had a TB test/screening dated: 5/12/2023.

Plan of Correction: Ensure that all staff have TB tests done before they start. Staff #1 will get a TB screening this weekend.

Standard #: 8VAC20-780-40-G
Description: Based on review of five child records, the facility failed to meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia. Evidence: the record for child #1, child #3 and child #4 did not contain documentation of proof of child identity and age.

Plan of Correction: Ensure that all children have birth certificates before they start.

Standard #: 8VAC20-780-70
Description: Based on review of six staff records, it was determined that not all records contained all required information. Evidence:
1) The record for staff #2 did not contain documentation of two reference checks as required;
2) The records for staff #2, staff #3, and staff #4 did not contain documentation to demonstrate that the individual possesses the education or certification required by the (lead teacher) job position;
3) The records for staff #1 and staff #5 did not contain documentation of orientation training as required.

Plan of Correction: 1. Reference letter will be given to staff. 2) Lead teacher program will be put together with required training. 3. Orientation hours will be conducted.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: In eight out of eight classrooms, there was a bottle of bleach water solution in in unlocked cabinet.

Plan of Correction: Lock all bleach bottles in the closet and changing tables.

Standard #: 8VAC20-780-550-F
Description: Based on review of documentation, not all lockdown drills were practiced at least annually as required. Evidence: The last documented lockdown drill was conducted on: 8/11/2021.

Plan of Correction: Set reminders on my desktop calendar for lockdown drills.
