Comments:
A monitoring inspection was conducted on October 5, 2023 with the center director and staff. There were 146 children present, ranging in ages from 6 months to12 years, with 21 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A
Description: Based on review of ten staff records, the facility failed to ensure that every employee repeat a background check in accordance with the Code of Virginia every five years.
Evidence: The most recent search of the central registry (CPS check) for Staff 1 was dated 09/27/2018; there was not a repeat CPS check obtained within 5 years. The most recent search of the central registry for Staff 7 was dated 9/11/2018; there was not a repeat CPS check obtained within 5 years.

Plan of Correction: Background checks will be renewed every 5 years as required.

Standard #: 22.1-289.035-B-4
Description: Based on review of ten staff records, the center failed to ensure that out of state background checks were conducted as required. Evidence:
Staff 3 (date of employment 7/11/2023) indicated on the sworn disclosure statement dated 7/14/2023 that they had lived outside the state of Virginia within the past five years. There was no documentation to support that the required out of state search of the child abuse and neglect registry (CPS check) had been submitted within 30 days of employment as required. (Connecticut). The director verified that an out of state search of the central registry was not completed for Staff 3.
Staff 4 (date of employment 4/10/2023) indicated on the sworn disclosure statement dated 4/10/2023 that they had lived outside the state of Virginia within the past five years. There was no documentation to support that the required out of state search of the child abuse and neglect registry (CPS check) had been submitted within 30 days of employment as required. (Texas). The director verified that an out of state search of the central registry was not completed for Staff 4.

Plan of Correction: The center completed out of state criminal record checks, but did not conduct out of state CPS checks.

Standard #: 8VAC20-780-510-I
Description: Based on review of 13 medications, the facility failed to have written authorization from the parent to administer medication. Evidence: There was no written parent/guardian or physician authorization to administer 3 medications to 2 children. Child A and Child G had emergency medications on site, but did not have a completed medication authorization.

Plan of Correction: Medication authorizations will obtained or the medications will be returned.
