Comments:
An unannounced complaint inspection was conducted on 10/17/2023 based on a complaint received on 10/12/2023 regarding forbidden actions and special care provisions and emergencies. The information gathered during the inspection determined the complaint to be not valid. Staff were interviewed and a total of 3 children?s records, and 4 staff records were reviewed. The inspector arrived for the inspection at approximately 7:25 am and departed approximately 8:40 am.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations unrelated to the complaint and were documented on the violation notice issued to the provider.

Description: REPEAT VIOLATION
Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.

Evidence:
1. The record for Child 1 did not contain the following information: first and last day of attendance.
2. The record for Child 2 did not contain the following information: first day of attendance.
3. The record for Child 3 did not contain the following information: first day of attendance.

Description: Based on observation and record review, the center failed to ensure during stated hours of operation, there shall always be on premises and on field trip when one or more children are present one staff member who meets the qualification of a program leader or program director and an immediately available staff member, or volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them.

Evidence:
1. Upon the inspector?s arrival to the center on 10/17/2023 at approximately 7:25 am, Staff 1 was the only staff member present and there were 7 children in care, ages 18 months ? 4 years old. Staff 1 confirmed the first child in care arrived at approximately 7:00 am on 10/17/2023.
2. There were a total 11 children in care ages 18 months ? 4 years old, when Staff 2 and 3 arrived at approximately 7:55 am.

Plan of Correction: Staff schedules will be adjusted, and a new hire will start on 11/1/2023.

Description: Based on observation and record review, the center failed to ensure that the required staff to child ratio shall be maintained for 16 months up to 24 months.

Evidence: There were 11 children observed with 1 staff member. The youngest child was 18 months old. The required staff ratio for this group of children was 1:5. Three staff would be required.

Plan of Correction: Staff schedules will be adjusted, and a new hire will start on 11/1/2023.

Description: Based on observation, the center failed to ensure that there shall be at least two people who are currently certified in CPR and First Aid on the premises during the center?s hours of operation, on fieldtrips and wherever children are in care.

Evidence: Staff 1 was the only staff on premises with a current certification in CPR and First Aid from approximately 7:00 am until approximately 7:55 am on 10/17/2023.

Plan of Correction: Staff schedules will be adjusted, and a new hire will start on 11/1/2023.
