Comments:
An unannounced monitoring inspection was conducted on 10/12/2023 and concluded with an electronic record review on 10/18/2023. There were 18 children, ages 5 years - 9 years, and 4 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. The children were observed during snack time and homework time. A total of 5 children?s records, 4 staff records and 2 medications were reviewed. The inspector discussed the following with the provider: sanitizing and children?s records. The inspector arrived for the inspection at approximately 2:50 pm and departed at approximately 4:05 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review and staff interviews, the center failed to obtain documentation that each child has received the immunization required by the State Board of Health before the child can attend the center.

Evidence:
1. The record for Child 1 documented the first date of attendance as 08/16/2023. No immunization record was available for review. Staff 3 confirmed no immunization record was available for review.
2. The record for Child 2 documented the first date of attendance as 08/16/2023. No immunization record was available for review. Staff 3 confirmed no immunization record was available for review.

Plan of Correction: Provider will obtain information from parent.

Standard #: 8VAC20-780-140-A
Description: Based on record review and staff interviews, the center failed to obtain documentation that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.

1. Evidence: The record for Child 1 documented the first date of attendance as 08/16/2023. No physical examination was available for review. Staff 3 confirmed no physical examination was available for review.
2. The record for Child 2 documented the first date of attendance as 08/16/2023. No physical examination was available for review. Staff 3 confirmed no physical examination was available for review.

Standard #: 8VAC20-780-500-A
Description: Based on observations, the center failed to ensure that children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Evidence: Children were observed returning to activities before washing their hands after the afternoon snack.

Plan of Correction: Provider will ensure children wash hands or use a disposable wipe before and after meals/snacks before beginning other activities.

Standard #: 8VAC20-780-510-L
Description: Based on observations, the center failed to ensure that medication, except for those prescriptions designated otherwise by written physician?s order, including refrigerated medication and staff personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Medication 1 was observed in the unlocked top drawer of a storage unit on top of a filing cabinet in unlocked storage room.
2. Medication 2 was observed in the unlocked top drawer of a storage unit on top of a filing cabinet in unlocked storage room.

Plan of Correction: Provider will lock medication.
