Comments:
A monitoring inspection was conducted on 10:10 AM from approximately 10:10 AM to 1:30 PM. There were 115 children present in the center, ranging in ages from infant to preschool, with 22 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Description: Based on record review and interview, the center did not have documentation of an updated fingerprint-based criminal history record finding every five years from the date of the most recent

Evidence:
The record for staff #10 (FP: 8/16/18) had documentation of an updated fingerprint-based criminal history record finding on 8/31/23. An updated fingerprint-based criminal history record was required by 8/16/23.

Description: REPEAT VIOLATION
Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
On the toddler playground, the wooden house had screws protruding from the wood. In the toddler room, the square climber has torn edges showing foam and stitching.

Description: Based on observations and interviews, the facility failed to maintain compliance with this standard as required relating to children being in sight and sound of a staff member.

Evidence:
1. In the gym, there was no staff present with the soccer shots coach when he had a group o eight children doing soccer lessons. The time frame was approximately 10:20 AM to 10:30 AM.
2. The administrator stated that staff were not present during soccer shots.
