Provided consultation on standard:
1) 50A: Staff and children?s records shall be treated confidentially; the Supplemental Form is not to be posted as it contains confidential information
2) 60A8: A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction ? to be kept on site
3) 550F: lockdown procedures shall be practiced at least annually; Facility should consult with emergency personnel regarding procedures for this location
4) Discussed emergency medications for children with diagnosed food allergies.

Comments:
An unannounced monitoring inspection was conducted from 3:45 - 4:15 pm with staff. There were ten children, ranging from five years to nine years of age, present with two staff supervising. Children were observed at table time coloring. Two onsite staff records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, and the required posted information. It was reported that there were no medications on site, however, staff on site have current Medication Administration Training (MAT) certification. Two staff records were reviewed off site at the YMCA office on 9/6/2023. Two child records were reviewed virtually.

Description: Based on review of documentation, the facility failed to maintain a current written list of all children?s allergies, sensitivities and dietary restrictions. Evidence: the written allergy list did not contain child #2, with a documented food allergy/sensitivity.

Description: Based on review of two child records, not all records contained all required information. Evidence: the record for child #1 did not contain a work phone number for dad; the record for child #2 did not contain the name, address and phone number for a second emergency contact to call in an emergency.
