8VAC20-780-40M The list of children?s allergies should be kept in each area where children are present.
8VAC20-780-160A TB screenings should be completed prior to employment and coming in contact with children.
8VAC20-780-240B Staff orientation should include all topics listed in the standards.

Comments:
An unannounced renewal inspection was conducted on 10/23/2023 from 10:30am to 1:45pm. There were 38 children ages 3 years old to age 5 years old supervised by 6 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 5 children?s records, 4 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in lunch, reading and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on review of 5 staff records, the center did not obtain documentation of the results of a national fingerprint background check prior to date of hire.

Evidence: The record for Staff #4 (Date of Hire 8/14/23) and Staff #5 (Date of Hire 8/14/23), did not contain documentation of a fingerprint background check completed prior to their date of hire. The fingerprint background checks for both Staff #4 and Staff #5 were completed on 8/31/23.

Plan of Correction: All appropriate documentation will be obtained before the official DOE date of hire.

Standard #: 8VAC20-770-60-B
Description: Based on review of 5 staff records, the center did not obtain documentation of a completed sworn disclosure statement for each staff prior to date of hire.

Evidence: The record for Staff #5 (date of hire 8/14/23) did not contain documentation of a completed sworn disclosure statement prior to the date of hire. The sworn statement for Staff #5 was completed on 8/15/23.

Plan of Correction: All appropriate documentation will be obtained before the official DOE date of hire.

Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.


Evidence: During inspection in the Atrium classroom (ages 3-4), there were 8 children present, but the written record reflected there were 9 children present.

Plan of Correction: Teachers/Staff have been made aware of the policy to sign children in and out of the classroom as well as the school.

Standard #: 8VAC20-780-560-F
Description: Based on observation and staff interview, the center did not ensure they follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

Evidence: The school snack menu for the current week did not include a second component for each snack in the morning or afternoon.

Plan of Correction: The school snack menu has been updated.
