Comments:
An unannounced monitoring inspection was conducted on 10/26/2023 and concluded with an electronic record review on 10/26/2023. There were 13 children, ages 3 years- 4 years, 3 staff members providing direct care and supervision, and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during mealtime, teacher-directed activities, and outdoor time. A total of 5 children?s records, and 3 staff records were reviewed. The inspector arrived for the inspection at approximately 9:00 am and departed at approximately 10:15 am.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation, and measurements, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
1. The resilient surfacing (pea gravel) in the fall zone of the red dome climbing equipment (approximately 48 inches tall) measured approximately 3 inches on the outer edge.
2. The resilient surfacing (pea gravel) in the fall zone of the yellow/red dome climbing equipment (approximately 48 inches tall) measured approximately 3 inches on the outer edge.
3. The resilient surfacing (pea gravel) in the fall zone of the climbing equipment with yellow slide measured approximately 2 inches around the climbing platform and less than 1 inch at the base of the slide. Ground was visible at the base of the slide.
4. The resilient surfacing (pea gravel) in the fall zone of the climbing equipment with 2 purple slides measured approximately 2 inches around the climbing platforms and less than 1 inch at the base of the slides. Ground was visible at the base of both slides.
5. The resilient surfacing (pea gravel) in the fall zone of the climbing equipment with the horizontal ladder (approximately 66 inches tall) measured approximately 2 inches.

Plan of Correction: Provider will obtain additional surfacing.
