Comments:
An unannounced renewal inspection was conducted on November 2, 2023, from 9:45am-1:30pm. At the time of entrance, 19 children were in care with 5 supervising staff members present. The inspector reviewed 5 children's records and five staff records. Children were observed during playground time, reading books, toy play, and other group activities. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

The facility is reminded that a complete renewal application must be submitted prior to expiration of the license on November 22, 2023. Renewal applications should be submitted through the online portal available on the public website at childcareva.com.

Violations:
Standard #: 22.1-289.035-B-4
Description: Repeat Violation
Based on record review and interview ,the child day center did not obtain a copy of the results of a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #4's record (Date of hire-09/05/2023) contains documentation of a sworn statement stating that they lived in the state of New Jersey in the preceding five years. Staff #4's record did not contain documentation of results of a search of the child abuse and neglect registry from the state of New Jersey.
2. Staff #5 confirmed that search results of a search of the child abuse and neglect registry from the state of new jersey was not available at the time of the inspection.

Plan of Correction: A search was conducted for the staff member. Management followed up with New Jersey to obtain the results of the out of state cps search.

Standard #: 8VAC20-780-520-C
Description: Based on observation and documentation review, the requirements for diaper ointment were not met.
Evidence:
1.Written parent authorization of diaper ointment noting any known adverse reactions were not obtained for Child #1, Child #2, and Child #3.
2. In the infant classroom, a container of diaper ointment did not contain Child #2's name on the container.
3. In the early preschool classroom, a container of diaper ointment did not contain Child #1's name on the container.

Plan of Correction: The diaper ointments were labeled with the children's names and authorization forms were printed for the parents to complete.

Standard #: 8VAC20-780-550-P
Description: Based on documentation review, three of ten accident reports reviewed did not contain required information.
Evidence:
1. Three accidents reports did not contain the time the parents were notified.

Plan of Correction: Moving forward, staff will be retrained on completing accident reports to ensure all information was captured on the form.
