Technical Assistance:
Technical Assistance was provided to the Assistant Director regarding mulch, infant sheets provided from home, ratio of youngest child, diaper cream storage, and timing of tuberculosis screenings.

Comments:
An unannounced monitoring inspection was conducted on 11/1/23 from 10:10am to 2:10pm with the Assistant Director. There were 84 children in care, ranging in age from four-months to five-years-old, supervised by 21 staff. The infants were observed going for buggy rides around the inside of the center, looking at books, having diapers changed, and having free play. Older children were observed having free play, playing in the ?Ball Room?, listening to the teacher read a book, playing with blocks and washing hands for lunch, eating lunch, and rotating through structured centers. Six child records and six staff records were reviewed. The Center has 50 staff with current certification in CPR and First Aid, and 50 staff trained in Daily Health Observation Training. A sample of 11 medications and authorization forms were reviewed and the center has eight staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 1/10/23 and the most recent Health Inspection on file was dated 3/2/23. Lunch served was turkey and cheese wraps, mixed vegetables, pineapple, and milk. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 11/14/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of six staff records, the program did not obtain documentation of the results of a search of the central registry from each state that a staff member resided in within the past five years, within 30 days of employment. Evidence: The record for Staff #3, with an employment date of 9/18/23, contained documentation that the staff member had resided in West Virginia within the past five years. There was no documentation of the results of a search of the central registry from West Virginia on record for the staff member.

Plan of Correction: Staff member had been on medical leave for several weeks and unable to complete form. Staff has returned to center and out of state central registry has been completed and priority mailed to the appropriate agency.

Standard #: 8VAC20-780-130-A
Description: Based on review of six child records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: The record for Child C, with a start date of 3/2/22, contained documentation of immunizations dated 6/20/22. The record did not contain documentation of immunizations from prior to 3/2/22. The record for Child D, with a start date of 12/28/22, contained documentation of immunizations dated 3/31/23. The record did not contain documentation of immunizations from prior to 12/28/22.

Plan of Correction: Policy has been put in place that children can not start attending the center until all required documents are turned in and student file is complete.

Standard #: 8VAC20-780-80-A
Description: Based on review of documentation and interview, the center did not ensure that for each group of children, a written record of daily attendance was maintained that documented the arrival and departure of each child in care as it occurred. Evidence: 1. On 11/1/23 at approximately 11:42am, there were 12 children observed in the ?Threes 614? classroom. The attendance record contained documentation of 13 children present. Staff stated that a child had moved to another classroom around 9:00am. 2. On 11/1/23 at approximately 11:50am, there were 10 children observed in the ?Threes 617? classroom. The attendance record contained documentation of 11 children present. Staff stated that a child had moved to another classroom around 8:00am.

Plan of Correction: Both teachers were counseled on signing students in and out on the attendance when they are moved from one classroom to another.

Standard #: 8VAC20-780-440-I
Description: Based on observation and interview, the center did not ensure that cribs were placed where objects outside the crib, such as cords, are not within reach of infants. Evidence: On 11/1/23 an ?Alexa Dot? device was observed in the corner of an unoccupied, but assigned, infant crib. The cord was plugged into the device and the wall outlet and the device, along with the cord, was sitting inside of the crib. Staff reported that it had been moved there the day before.

Plan of Correction: Staff in classroom were counseled that items should never be kept in cribs other than babies in cribs. Disciplinary action was taken with staff responsible for violation.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure that disposable diapers were disposed of in a leakproof or plastic-lined storage system that was either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touched an exterior surface of the storage system during disposal. Evidence: On 11/1/23, disposable diapers/pull-ups were observed in an open trashcan that did not have a lid or a trash can liner and was not foot-operated.

Plan of Correction: Foot operated trashcans were purchased for both classrooms and placed in each bathroom.
