An unannounced investigation was initiated on 11/1/23 in response to a complaint received in the licensing office regarding an allegation related to preventing the spread of disease. Documentation was reviewed and staff were interviewed. The investigation was continued off-site with more interviews. There was insufficient evidence to support the allegation and no violations were cited. An exit interview was conducted with program management on 11/8/23.
