Comments:
An unannounced on-site renewal inspection was conducted on 11/06/2023. There were 28 children, ages 17 months-5 years, and 9 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, lunch and nap. A total of 5 children?s records, 5 staff record were reviewed. The inspector discussed the following with staff: medication, , playground safety. The inspector arrived at 10:30AM and departed at 1 PM.

Violations:
Standard #: 8VAC20-770-70-A
Description: Based on review of ten staff records, the center failed to ensure that all records were complete per the standard.

Evidence:

Staff # 3 had a hire date of 05/31/2023 and the two required reference were dated 06/02/2023. The reference checks are required prior to employment.

Plan of Correction: In the future all references and items required prior to employment will be completed as required.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that all areas and equipment of the center were maintained in a clean, safe and operable condition.

Evidence:

The outside play area has exposed siding on the building that has visible cracks and missing pieces. These cracks are accessible to the children and have sharp edges.

Plan of Correction: The owner will have the damaged siding replaced and will monitor.

Standard #: 8VAC20-780-510-P
Description: Based on observation and review of medications and authorizations, the center failed to ensure that all medications were returned to the parent or guardian within 14 days after the authorization expired or disposed of by the center.

Evidence:

Medication authorization for Child # 6 to administer nonprescription medication was signed by the parent on 10/02/2023 and expired on 10/16/2023. The medication had not been returned to the parent. The medication was observed in the center on the day of the investigation.

Plan of Correction: The medication will be returned to the parent today.
