Comments:
A monitoring inspection was conducted on 11/7/2023. The inspector arrived at 9:15 AM and departed at 12:00 PM. There were 43 children present, ranging in ages from 4 years to school age, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with staff 1 during the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based upon review of staff records, the facility has not ensured that each staff member submits documentation of a negative tuberculosis screening that was completed within the last 30 calendar days of the date of employment.
Evidence:
According to the record provided, staff 4 was employed on 9/11/2023. The tuberculosis screening provided was dated 11/6/2023 which is not within 30 days prior to employment.

Plan of Correction: The facility responded with the following:
We have obtained the TB test.

Standard #: 8VAC20-780-40-E
Description: Based upon observation and review of records, the facility has not ensured that operational; responsibilities of the licensee shall include ensuring that the center's activities and services are maintained in compliance with the terms of the current license issued by the department.
Evidence:
Child 1, four years old, was in care during the inspection. The facility is licensed to care for children from ages 5 through 12 years only.

Plan of Correction: The facility responded with the following:
We are working on getting another site licensed for four year olds.

Standard #: 8VAC20-780-70
Description: Based upon review of staff records, the facility has not ensured that each staff record includes documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence:
The reference checks provided for staff 4, employed on 9/11/2023, are dated 10/30/2023, which is not before employment of staff 4.

Plan of Correction: The facility responded with the following:
References completed for staff.

Standard #: 8VAC20-780-240-A
Description: Based upon review of staff records, the facility has not ensured that staff complete the Virginia Department of Education (VDOE)-sponsored orientation course within 90 calendar days of employment.
Evidence:
The record provided for staff 3, employed since 11/1/2021, did not include documentation that staff 3 completed the ten hour VDOE-sponsored preservice orientation course.

Plan of Correction: The facility responded with the following:
Site orientation have been completed for staff.

Standard #: 8VAC20-780-560-N
Description: Based upon observation, the facility has not ensured that food is stored in a clean and sanitary manner.
Evidence:
The licensing inspector observed a partially eaten chicken sandwich is an open clear wrapped package on top of a coat in a basket on the floor where children's belongings are stored. Administrative staff brought it to the attention of the staff on duty that the sandwich had been opened, partially eaten and was exposed to air and other potential contaminants.

Plan of Correction: The facility responded with the following:
Coached the staff on making sure lunches are stored away.
